The individual named above (referred to as “I” or “me”or “participant”) desires to participate in voluntary and recreational basketball at George Southard Elementary School, Lockport, NY 14094 (the “Activity”), and rented by Freedom Church (“Activity Sponsor”). In consideration of being permitted to participate in the Activity and in recognition of the Location and Activity Sponsor’s reliance hereon, I agree to all the terms and conditions set forth in this instrument (this “Release”).
