2026-27 FCHC Co-Op Final Registration

Welcome to Freedom Church's Home School Co-op for the 2025-26 School Year!

Thursdays from 9:00am-12:30pm.

We are excited to welcome your family! Please complete the following form to finish your registration and submit your payment.

Please register all parents who will attend, regardless of how frequently they plan to be present. For example, if Mom Campbell will regularly assist and primarily bring the children, she must be registered. If Dad Campbell works full time but may occasionally adjust his schedule to attend co-op, he must also be registered and complete the required background check and MinistrySafe training. Any parent who may be present during co-op hours is required to complete registration and all safety requirements.

Please register total number of children that you will be bringing (0 mos.- 12th grade). For example, Mom Campbell has an eighth grader, a third grader, a three year old and a 5 month old. She will register as four children and tuition fee will be $300 for Campbell Family.

Statment of Faith - Required

Participation in Freedom Church Homeschool Co-Op (FCHC) requires agreement with and adherence to our Statement of Faith and Honor Code. These documents outline our core beliefs, biblical convictions, and expectations for conduct within our church community. By registering, you acknowledge that you have read and agree to live in alignment with Freedom Church’s Statement of Faith and Honor Code. Ministry Safe Training & Background Check

To participate in FCHC, all adults who will attend or be present in the building during operating hours are required to complete a background check and MinistrySafe training. Because at least one parent is required to attend and teach or assist, a minimum of one parent per family must complete the training and provide consent for a background check. Please indicate your consent in the fields below.

SOCIAL MEDIA & PHOTO RELEASE POLICY

During Freedom Church Homeschool Co-op (FCHC), designated leaders (Kerith, Jennie, and Megan W.) may take photos and videos of children participating in various activities.

By signing below, you grant Freedom Church permission to use photos and/or videos of your child(ren) taken during FCHC for purposes including, but not limited to, the FCHC yearbook, church publications, news articles, the church website, Facebook and Instagram pages, and promotional materials or advertisements.

All photos and videos will be used without identifying names.

DIAPER POLICY WAIVER

By signing this document, I acknowledge and agree that, as a teacher participating in the Freedom Church Homeschool Co-op (FCHC), I am voluntarily permitting my child(ren) listed below to be supervised in the FCHC Nursery during regular co-op operating hours. I further consent to my child(ren)’s diaper being changed, when necessary, by an approved Freedom Church Homeschool Co-op leader.

If I prefer that other adults not change my child’s diaper, I understand that it is my responsibility to communicate directly with the homeschool co-op leaders, Jennie Campbell and Kerith McDonald, to make appropriate arrangements. I acknowledge that all FCHC Nursery workers have completed MinistrySafe training and have undergone a background check.

By signing below, I release and hold harmless Freedom Church and the Freedom Church Homeschool Co-op from liability related to the routine care and supervision of my child(ren) during regular Nursery operating hours, except in cases of gross negligence or willful misconduct.
